Current Concept of Esophageal achalasia


Abstract
Esophageal achalasia is a disease of unknown cause which is characterized by disordered esophageal motility. Although many etiologic theories have been proposed, there is general agreement that it is a neuromuscular disorder. In general, achalastic patients reveal degeneration or absence of the esophageal myenteric ganglion cells, however, these findings are not consistent one, and in dog and cat as well.
There are mounting evidences to support the conclusion, that the primary site of achalasia is central, by studying Wallerian degeneration of the peripheral vagal nerve and decrement of vagal motor nucleal cell in number. It is suggested that an extraesophageal vagal lesion is the primary site of involvement in esophageal achalasia and that the esophageal changes are secondary.
